What We Offer
Any provider at your practice can submit referrals via EHR or fax. Include the following:
- Patient demographics & insurance details
- Signed order & supporting office notes
- Relevant lab results & contact information for the referring provider

Once received, our team will confirm and review the referral for accuracy and to confirm the order can be filled at Ascend Infusion.. If the requested drug is new to us, we’ll ensure safety protocols and staff training are in place.

We verify insurance coverage, submit prior authorization requests, and collaborate with your office to handle any issues related to approvals or denials

Our experienced infusion nurses review patients’ medical history, lab values, and orders before scheduling to ensure optimal care.

Our coordinator will contact the patient, schedule their visit, and guide them on what to expect—both for their new patient office visit and first infusion appointment. We’ll also review financial responsibilities, including any co-pay card options.

Patients will have an initial consultation to review relevant medical history. If needed, this can be scheduled on the same day as the first infusion.

The patient will sign a consent form on the day of the infusion. Our expert team will administer the therapy according to the protocols, closely monitoring vital signs.

After the first infusion, the Ascend Infusion team will contact the patient to see how they are feeling and to schedule their next appointment.

Our streamlined referral process ensures a seamless experience for you and your patients.

We see ourselves as trusted partners to each physician who refers their patients to our care.
What Makes Ascend Infusion Different
Infusion Flow Sheet Management:
After an infusion, the flow sheet, signed off by the infusion nurse and supervising physician, is faxed to the referring physician for their records. This ensures proper documentation and continuity of care.

Ongoing Care Tracking: Ascend Infusion monitors each patient’s journey, ensuring that all necessary documentation is in place for continued treatment.
Key Measures Tracked:
Annual Office Visits: Patients are tracked to ensure they are scheduled for any required yearly visits.
Updated Medical Records: Ascend Infusion ensures that updated records from the referring provider are obtained before each infusion or as required by treatment protocol.
